Registering with Italy's Public Health Service in Rome: Who Can Do It and How
A free GP, specialist visits at reduced cost, and prescription medicines: here's how to sign up with the SSN at your local ASL in Rome.
In a nutshell
SSN (Servizio Sanitario Nazionale -- Italy's national health service) is Italy's public healthcare system. Once registered, you get a free GP (general practitioner), access to public hospitals, specialist appointments at a reduced co-payment (ticket), and subsidised medicines. Rome has six ASL (Azienda Sanitaria Locale -- your local public-health authority), and which one covers you depends on which district (Municipio) you live in. Registration is free for those who qualify and is done on the spot at a walk-in counter.

Which ASL covers you
Rome is divided into six health authorities. Find yours first:
- ASL Roma 1: Municipi I, II, III, XIII, XIV, XV -- Headquarters: Borgo Santo Spirito 3, tel. 06 68351
- ASL Roma 2: Municipi IV, V, VI, VII, VIII, IX -- Headquarters: Via Maria Brighenti 23, tel. 06 51001
- ASL Roma 3: Municipi X, XI, XII + Fiumicino -- Headquarters: Via Casal Bernocchi 73, tel. 06 56481
- ASL Roma 4, 5, 6: Municipalities in the wider Metropolitan City of Rome, outside the city proper

Who qualifies for free registration
Mandatory and free registration applies to:
- Italian citizens resident or domiciled in Rome
- EU citizens with official residence (residenza anagrafica) in Italy, or with an employment contract in Italy
- Non-EU citizens holding a permesso di soggiorno (residence permit for non-EU citizens) for work, family reunification, asylum, subsidiary protection, special cases, long-term residency, or pregnancy
- Asylum seekers from the moment they file their application at the Questura (police headquarters -- also issues residence permits), as documented by the C3 interview record
- Newborns born in Italy: automatic registration from birth up to 6 months
- Dependent family members of an SSN enrollee
- Detainees, including undocumented foreigners
If none of the above applies to you but you are legally in Italy, you can still access the SSN through voluntary paid registration.
Documents you need at the counter
Bring originals and photocopies of everything.
Italian citizens and EU residents:
- Valid ID document
- Codice Fiscale
- Self-declaration of residence (autocertificazione) or receipt of the residency application at the Municipio
EU workers (not yet registered as residents):
- ID document or passport
- Codice Fiscale
- Employment contract or INPS (Italy's social-security agency -- pensions, unemployment, family benefits) registration / Partita IVA
Non-EU citizens with a permesso di soggiorno:
- Passport
- Valid permesso di soggiorno, or the postal receipt (ricevuta postale) from the renewal kit
- Codice Fiscale
- Self-declaration of residence or address (the address registered with the Questura is accepted)
Asylum seekers:
- C3 record or temporary asylum-seeker permit
- Codice Fiscale (provisional numeric code is fine)
- Address (even a reception centre address)
Minors:
- Parent's ID and the child's Codice Fiscale
- Child's birth certificate or passport
How registration works
The process is straightforward and in most cases requires no appointment.
Go to the CUP / Anagrafe Sanitaria desk in your ASL district. ASL Roma 1, 2 and 3 generally operate on a walk-in ticket system with no booking needed. ASL Roma 4, 5 and 6 often ask you to call ahead or book online.
Typical hours are Monday-Friday 8:00-12:30, with some counters also open Tuesday and Thursday afternoons 14:30-16:30.
At the counter you complete the registration form and choose your GP (medico di medicina generale) from the list available in your area. Each GP can have no more than 1,500 registered patients (1,000 for paediatricians). Children up to 14 years old are registered with a paediatrician (Pediatra di Libera Scelta, PLS).
Before you leave you'll receive:
- The SSN registration document (foglio di iscrizione), valid immediately -- you can see your GP that day
- Your regional patient code (codice assistito)
- Confirmation of the expiry date (open-ended for Italian and EU residents; tied to the permit expiry for non-EU citizens)
The plastic Tessera Sanitaria will arrive by post within 30-60 days. Don't wait for it before visiting your GP -- the registration document is sufficient.
Mistakes to avoid
- Waiting for the plastic card before seeing a doctor. The registration document issued at the counter is valid from day one. Take it with you along with your Codice Fiscale.
- Not updating your address with the ASL. If you move and don't notify the ASL, any renewal card will go to your old address.
- Thinking an expired permit blocks everything. If you have the postal receipt from your renewal kit, the ASL accepts it as a valid document. Your SSN registration continues without interruption.
Special cases
Non-EU resident without registered residency but with a valid permesso: ASL Roma 1, 2 and 3 accept a health-domicile declaration (domicilio sanitario). You can register even without formal residency, using the address where you actually live.
EU student: You can use the EHIC (European Health Insurance Card, called TEAM in Italy) from your home country for essential care, or register voluntarily with the SSN for a dedicated annual contribution if you want a full Italian GP.
Posted EU workers: If you're insured in your country of origin, bring the S1 form issued by your home country's social-security institution. The ASL will register you free of charge.
Newborns: Parents must choose a paediatrician at the ASL counter within the first 6 months. You'll need the baby's Codice Fiscale before you can do this.
Already enrolled in another region: You need to cancel your registration with the ASL in the other region first -- or the Rome ASL counter can do it automatically. You cannot be enrolled in two ASL at the same time.
